Included in these 5 documents is a waiver form, medical form, gear-list, checklist and detailed expedition notes. Please fill in the medical and waiver form and send back to us at your earliest convenience. Please, also, make sure to have a look at the gear-list and checklist and have all of this organised before you leave for Nepal.
